We understand that life doesn't always fit between 8am-4pm, so we offer a secure online portal for our current patients. The portal allows you to look up appointments, view statements, pay online, fill out forms and send them directly to us, even watch dental education videos. Click below to login to your portal.
Not everyone likes to do everything online and sometimes it can be tricky. If you prefer to print out the New Patient Health History Forms, click below. Then be sure to send them to us before your appointment or come a little early and bring them with you to our office. We look forward to meeting you!
Many of our patients have dental insurance, and if you have a PPO where you can choose the dentist you can go to, then good news, you have coverage at our office! We will submit your dental insurance claims to your carrier, as a courtesy to you, our patient. However, our loyalty always stays with you, our patient as we are not contracted with any dental insurance companies. In turn, any balance not paid by your insurance company is your responsibility. We do offer payment options as we understand that certain procedures can be an investment in your health. Please read below regarding our payment policies.
In an effort to keep dental costs down while maintaining a high level of professional care, we have established the following payment options for the use of our patients.
Treatment may be paid as follows:
1. Prepayment or payment at the time of treatment with a courtesy off.
2. Payment of balance in full within 10 days of statement date.
3. Use of your Visa, Mastercard, Discover or American Express
Extended payments are available upon special request. Please speak with our Treatment Coordinator or Financial Coordinator for details and options available to you.
Accounts outstanding more than 60 days from treatment date will bear interest at a rate of 1.5% per month (or 18% annum).
All major treatment and treatment involving a laboratory procedure will require an appropriate down payment and a financial agreement prior to services being rendered.
We also offer payment plans through CareCredit. Care Credit offers up to 12 months interest-free or extended (2- 5 year) payment plans! To apply for Care Credit, click on the button below.
We love seeing our patients, we truly do! We always want to respect your trust in us and your time as well. That is why we see patients by appointment only. Rest assured, if you ever have a dental emergency, call us right away and we will do our very best to see you as soon as possible. We also ask for your patience if we ever are running a few minutes behind due to seeing a patient with an emergency or who needed a little extra time or care that day. Know that when you are being cared for we will give you the same time and detailed attention that you deserve as well.

• Hand Sanitizing Stations throughout the office
• Additional daily cleanings of frequently touched surfaces
